Office hours are Monday through Friday 8 am – 5 pm at both the Fort Worth and Southlake locations. For new patient office appointments we ask that you bring all you medications, any radiology films in addition to the report, laboratory results and if you are on CPAP or BLPAP equipment please ALWAYS bring your equipment, including machine, cords and mask. We will request your medical records if you have a referring physician.
For follow-up office visits we ask that you notify the clinic staff of any changes in your medications, change in physicians and if you are on CPAP or BLPAP, please ALWAYS bring your equipment, including machine, cords and mask for download compliance.
We try to adhere to schedule appointment times, however there will be occasions where your appointment may be delayed. We understand your frustration and we do respect your time. However we are a subspecialty pulmonary and sleep medicine practice which is very different from a primary care or dental practice. Our patients are medically complex with multiple medical diagnoses and typically on multiple medications. Medical care of our patients is our priority and is unpredictable. We do our best to address all of each patient’s needs and answer their questions. Education is just as important as medications to medical care and these discussions can take time. We will try to notify you ahead of your scheduled time if there are any delays. If our delay conflicts with your schedule, please feel free to reschedule your appointment time. If you need test results, notify the receptionist, and we will either provider you with a written report or the physician will call you with the results within 24 hrs.
If you need to reschedule your appointment, please call to schedule at least 24 hrs. prior to your scheduled appointment time.
For inclement weather office closures or delays the office staff will contact you if the clinic office is unable to open or is delayed. If there is any question if the office is open, please call the office prior to leaving your house at 817-731-0230.
During normal office hours, phone calls received prior to 5 pm are returned on the same business day. If you have an emergency after 5 pm, please call our office phone number, 817-731-0230 and the answering service will notify the physician on call.
Please allow 48 business hours for prescription refills. You can request a prescription refill through the patient portal, calling the office or have our pharmacy send a refill request. Please note, prescription refills requested during the weekend or holiday will not be filled until the next business day.
We accept most insurance plans including Medicare. Please contact the office at 817-731-0230 if you have a particular question about your plan. Your insurance benefits will be verified prior to your appointment. Any copayment, co-insurance and/or deductible is due at the time of service. We accept: Visa, MasterCard, Check and Cash.
Effective 1/1/2016: Please note, with the change in health insurance payments all patients will be asked to keep credit card information on file in order to process any patient owed payments that were not collected at the time of service. You will be notified of the balance prior to processing the payment. If there is a question about your balance you will have 72 hrs. in which to call the office and notify us of the discrepancy otherwise the payment will be processed. If you elect not to keep credit card information on file you can pay your balance by viewing your statement on-line through the patient portal and either calling in or mailing your payment. If we have to send a paper statement, there will be a $25 statement fee assessed.
